Sports Physical and Immunization Records




NEW IMMUNIZATION REQUIREMENT
for students entering 7th grade
It is always recommended that you provide the school nurse with an updated immunization record when your child receives new vaccines.
Effective for the 2017-2018 school year, all students entering, advancing, or transferring into seventh grade will need to receive one dose of Tdap (tetanus, diphtheria, and pertussis) vaccine before attendance is allowed. This requirement is for seventh grade students only; grades 8-12 are not included in this rule change.
Documentation of the vaccine is required from your child’s physician. Updated immunizations can be faxed to: Attention: Brenda Bladen at 439-9958

SPORTS PHYSICALS
All students planning to participate in middle or high school sports are required to have proof of a current physical exam on file in the school nurses office. Sports physicals are considered valid for one calendar year. Scheduling your student’s physical exam in summer or fall would cover their participation in any school sport for that entire school year. Students may schedule a physical with their own physician and provide documentation to the school, or they can take advantage of the pre-season sports physicals conducted here in school before the start of the fall sports season. Practitioners from Kittery Family Practice/York Hospital conduct our school physicals. The cost is $5.00, paid in cash on the day of the physical.
SPORTS PHYSICALS at SHAPLEIGH SCHOOL will take place on TUESDAY, SEPT. 12TH FROM 7:45AM-11 AM. Our middle school sports physicals are offered during the first weeks of the school year, the time when many students first decide to participate in school sponsored sports. This is a very hectic time for students and families, so be on the lookout for the Pre-Participation Sports Exam forms during the first week of school. These forms MUST be completed, signed by a parent/guardian and returned to the school nurse before the Sept. 12th school physical date, in order for the student’s physical to be done.

As a reminder, if your student requires any medication to be given during the school day, an adult must deliver the medication to school. Please sign the Medication Consent form and have their physician sign the Prescription Medication Order. I can send you a hard copy or you can download and print them from kitteryschoolnurses.blogspot.com. This website is accessible from the Kittery Schools website under “nursing department”. It is a great site that contains useful forms and resources.
When the school year starts, please encourage your student to eat breakfast at home or at school and plan to send them in with a refillable water bottle and a snack to help them get through the day.
