From the Shapleigh Nurse’s Office
Dear Shapleigh Families,
I hope you all are having a wonderful summer and I’d like to extend a special hello to all of our new students coming from Mitchell and other school districts! I look forward to seeing everyone when school starts in September.
As summer starts to wind down, I ask that you start thinking about whether there have been any changes in your student’s health status that I should know about. These include things like asthma, allergies, or new medications. I can’t emphasize enough the value of having an allergy action plan or an asthma action plan on file for your student.
I am also requesting that a copy of any updated immunizations be faxed, emailed or sent in for their health file. The State of Maine is requiring students entering the 7th grade to receive their Tdap booster (prevents pertussis, diphtheria and tetanus) prior to their enrollment in September, so if you have not supplied that to us already, please ask their physician’s office to fax me their record.
For students entering the 6th -8th grades, we will be offering sports physicals on Tuesday, September 12th for $5.00 (see enclosed form). Students who are planning on joining a Shapleigh sports team will have to have an up to date, signed physical on file in their health record.
As a reminder, if your student requires any medication to be given during the school day, an adult must deliver the medication to school. Please sign the Medication Consent form and have their physician sign the Prescription Medication Order. I can send you a hard copy or you can download and print them from kitteryschoolnurses.blogspot.com. This website is accessible from the Kittery Schools website under “nursing department”. It is a great site that contains useful forms and resources.
When the school year starts, please encourage your student to eat breakfast at home or at school and plan to send them in with a refillable water bottle and a snack to help them get through the day.
